REQUIRED REPORT FOR ANY GRANTS RECEIVED

The Use of Grant Funds Report is a concise look at how funds were used and must be completed and returned by any organization receiving a CWC grant last year. If the organization applies for another grant in the current grant year, this Report MUST be submitted with the new Grant Application. If the organization does NOT reapply, this report must still be submitted independently by November 30. For any fields requesting dollar amounts, enter the amount only, without a dollar sign.
